About Direct Primary Care and Concierge Medicine

What is Direct Primary Care (DPC)?

Direct Primary Care is a membership-based model of primary care in which patients pay a membership fee directly to their physician, bypassing insurance for primary care services. In exchange, members receive unlimited visits, direct 24/7 physician access, and longer appointments.

Is this the same as concierge medicine?

At Total Access Medical, we provide concierge medicine through a direct primary care model, giving you straightforward, predictable access to your physician. Unlike some concierge practices that continue to bill insurance for office visits, potentially resulting in copays or deductible expenses, your Total Access Medical membership covers your visits with no additional charges.

What is the difference between DPC/concierge and my current insurance-based doctor?
Direct Primary Care (DPC) / Concierge MedicineInsurance-based practices
Stronger, more personal relationship due to more time and smaller panel sizeOften more transactional and limited in scope due to time constraints
Flat membership fee covering most primary care servicesFee-for-service model; costs billed to insurance and often include unpredictable co-pays and deductibles
Transparent, predictable costs with no hidden feesComplex billing with variable costs and surprise medical bills
Access to your physician directlyIt is rare that your doctor directly messages or calls you without an intermediary
Provides timely access with same-day or next-day appointments and flexible schedulingMany primary care doctors are inaccessible for timely appointments due to high patient volume and scheduling constraints
Exclusively see your primary care doctorMay have to see other providers who don’t know you
Longer, unhurried appointments (typically 30 minutes for acute visits, 1+ hour for more comprehensive visits)Shorter appointments (typically 15-20 minutes)
Smaller patient panel (300-600 patients per physician)Larger patient panel (2000-3000 patients per physician)
Little to no wait on the day of appointmentLonger wait times, often 30-45 minutes
Continuous, personalized, evidence-based management with a holistic and cost-effective approachCare varies widely, may be fragmented requiring multiple follow-up appointments due to insurance and time limitations
Do you take my insurance?

We do not directly bill any insurance companies. We work exclusively for our patients, not for insurance providers. Operating without insurance restrictions and boundaries allows us to provide you with the highest quality healthcare without compromising on personalized attention.

Do I still need health insurance?

Yes, we strongly recommend it. Your DPC membership provides comprehensive primary care but does not cover hospital stays, emergency care, specialist visits, or surgery. Additionally, you will utilize your insurance for laboratory testing, imaging tests, and medications ordered by your DPC provider. We strongly recommend maintaining a health insurance plan for these scenarios.

Is there an enrollment fee?

At Total Access Medical, we do not charge an enrollment fee.

Is there a long-term commitment?

There is a 12-month minimum commitment from your enrollment date. After that period, the membership is month-to-month. Month-to-month plans require 30 days written notice to cancel.

Can I use HSA funds to help pay for the membership fee?

Federal law changed in 2026 so that a qualifying direct primary care arrangement does not disqualify you from contributing to a Health Savings Account, and qualifying fees may be reimbursable from an HSA. These rules include limits on the monthly fee and on which services are covered, so whether your membership qualifies depends on your situation. Please check with your HSA administrator or tax advisor. Our office staff can provide a receipt or documentation of your membership fees.

What will I still use my insurance for?

You will continue to use your insurance for medications, laboratory testing, imaging tests, immunizations, specialist visits, and hospital care and emergency services.

What is integrative medicine?

Integrative medicine is a whole-person, healing-oriented approach to healthcare that blends conventional medical care with evidence-based complementary therapies such as lifestyle changes, nutrition, acupuncture, and mind-body medicine.
